In a flashback, Jimmy works in the mailroom at HHM. Jimmy asks Kim to open his bar exam results; he passes and she kisses him on the lips. Jimmy then shows Chuck his results and explains that he studied law via distance learning from the "University of American Samoa". Chuck seems to be happy for him. After, there is a small mailroom party to celebrate, but Howard tells Jimmy that HHM refuses to hire him as an attorney. In 2002, Hamlin holds a press conference to announce Craig Kettleman's plea deal, as Jimmy visits clients at a senior care home called Sandpiper Crossing. Jimmy visits a client, discovering that Sandpiper is overcharging her. Jimmy returns to Chuck's house, and points out fraud by Sandpiper Crossing. Chuck tells Jimmy he needs more information, Jimmy returns to Sandpiper, but is forcibly thrown out. Meanwhile, Mike receives a call from Stacey, asking him to babysit Kaylee. Later that night, Jimmy searches the Sandpiper's dumpster for shredded documents. He brings bags of the shredded documents to Chuck's house, but makes no progress and falls asleep. Chuck manages to put some of the documents together, which proves Sandpiper's guilt. Mike meanwhile spends the day with Kaylee, and is asked by Stacey what she should do with the money Matty accepted. Mike tells her to spend it and returns to the vet for a job. Jimmy meanwhile sets up a meeting with Sandpiper's lawyers at Chuck's house, and Chuck demands 20 million dollars from them. After, Chuck explains he sees a way to win which Jimmy is visibly ecstatic. The next day, Jimmy returns to Chuck's house with new client files and naps on his sofa. Chuck absentmindedly walks outside to Jimmy's car to grab some documents Jimmy forgot, and they stand outside in amazement.

Meredith dreams of a world in which her mother, Ellis Grey, never had Alzheimer's and instead had loving and supportive parents. This causes Meredith to be happy, which causes Seattle Grace to be completely different.

Meredith is mortified when her one night stand shows up at the hospital suffering from priapism and needing medical attention. Izzie, Addison and Derek are assigned to a case where a woman carrying quintuplets is overwhelmed by numerous medical issues as she navigates her high-risk pregnancy. Alex's shortcomings frustrate Izzie, especially when she catches Alex and Olivia having sex. On the other hand, Alex again shows his incompetence in treating one of his patients by administering the wrong dosage. Cristina reluctantly shows Preston where she lives.

Meredith and Derek have to use more than medicine when a young Asian woman's traditionalist, religious father forbids her to have a life-saving operation unless a shaman is called upon to "retrieve her souls". Meanwhile, George and Alex are forced to perform surgery in a trapped elevator in an attempt to save a patient's life in which George shows his potential while Alex shuns the challenge, and it is decision time for Derek and the two women in his life.

Cristina is still recovering from her surgery while receiving a visit from her mother. Not wanting to rest in bed, she uninvited diagnoses Burke and Izzie's patient with Munchhausen's syndrome. Eager to seem fine, Cristina have refused to mourn all day, but in the end, she breaks down. Addison gives Derek divorce papers to sign, but he can't make up his mind. Alex asks Izzie on a date, but finding out that he hasn't passed an exam makes him unable to make the most of it. Bailey treats a longtime patient with cystic fibrosis to whom she's emotionally attached.

Ellis Grey is brought into the hospital for possible diverticulitis, and her Alzheimer's condition becomes known to everyone. George is assigned to her case, but she refuses him to treat her, thinking he's her ex-husband. Alex and Meredith treat a young patient with extreme blushing. Izzie stands up for her and Alex's blossoming friendship to the others. To Preston's shock, Cristina collapses in the O.R. and Addison has to remove her Fallopian tube, causing her baby to be aborted. Meredith tells Derek that hating him is exhausting and that she doesn't want to do it anymore.
