A trauma counselor, Dr. Perkins, tries to help the hospital staff in the recovery and to assess each doctor's readiness to return to work, while Derek quickly returns to work with adrenaline.

Elena Gilbert and her brother Jeremy can't come to grips with the fact their parents have died. But nothing goes as planned for both of them when mysterious new student Stefan Salvatore attracts Elena's attention, leaving her ex-boyfriend Matt and her rival Caroline unaware of what's happening. After Matt's sister Vicki is bitten by something they believe to be an animal, it's discovered that Stefan is a vampire. Stefan (who does not feed on humans) realizes he is not the only vampire in town. Stefan's evil brother Damon enters the picture; they have not seen each other for 15 years.

After traces of blood linked back to a missing teen are found, the team re-investigates the 2006 case of a boy who went missing from a high school for the deaf. The victim is seen by both Lilly and the victim's girlfriend, who signs "I love you" to him.

It's Lucas and Lindsey's wedding day and Peyton must choose whether to intervene or lose her true love forever. Nathan uses the occasion to attempt a reconciliation with Haley, and the recently paroled Dan shows up for the family reunion. Carrie drops by and decides to take Jamie. Dan comes and saves the day. Carla and Elliot convince The Todd to admit to being gay, but contrary to what they had hoped, this does not change his personality for the better. Dr. Cox initially consoles J.D. but struggles to follow his own advice later on, when he finds out that Jill actually died of rabies and the infected organs went on to kill all the recipients.

When a series of deaths occur at Seattle Grace, the uncanny events bring out the doctors' superstitious sides. Four deaths in the morning implies that three more deaths are due for the day. This makes Izzie nervous about Denny's surgery, and a jealous Alex informs Denny of the likelihood of his death, prompting Izzie to completely end their relationship. Meanwhile, Derek and Addison discuss making a more permanent living arrangement, and Dr. Webber treats a former sponsor from his alcoholic's anonymous group. Izzie kisses Denny after he survives his surgery.
