Seven years ago, Hargreeves sends Luther on a mission that goes severely wrong. To save his life, Hargreeves injects Luther with a serum that also transforms his body into that of an ape. In the present, Allison and Luther find the deactivated Grace and assume the hit-men shut her off. Leonard and Vanya bond and agree to go to dinner. Meanwhile, Cha-cha and Hazel torture Klaus to get information about Five. When they begin to destroy Klaus's drugs, he gives up information, and after sobering up, he begins to see their dead victims. Five threatens the man from Meritech to get more information about the prosthetic eye. Cha-cha and Hazel, now aware of Five's investigation, go to the lab and torch it. Diego and Luther find Five passed out drunk. Patch goes to the motel to search for the two hit-men and frees Klaus but gets killed by Cha-cha. Klaus escapes through the vents with the briefcase. As he opens the briefcase, he time travels.

Damon believes if he sees Elena, all her memories of him will be restored, but Elena doesn't want to meet him. Alaric asks Elena to see if Jo knows about vampires, and she discovers that Jo is a witch and knows Elena and Alaric are vampires. Tripp captures Damon and try to drive Enzo and Damon over the Mystic Falls borders. Alaric tries to stop Tripp from crossing the Mystic Falls borders but gets dragged across it himself. While looking for Damon, Enzo, and Alaric, Elena decides she wants her memories back of Damon. When they cross the border, Enzo, Damon, and Alaric slowly start to die and get magic stripped off of them. Jo was following Alaric and rescues him, but he comes back alive a human. Stefan rescues Damon and Enzo and later on meets Caroline, who tells him she doesn't want to be friends anymore. The episode ends with Elena and Damon meeting in her dorm.

After his son comes to the clinic claiming to have received bruises on his back and front from a single fall skateboarding, a man is called in, but he shows violent behavior. He is determined to have a serious brain tumor causing emotional issues and impending death if not treated. After Michael convinces him to let him perform surgery, he confesses that he was involved in a hit and run with a red-haired woman around the same time and place where Anna was killed, causing mixed emotions for Michael. Meanwhile, a FDNY firefighter is having emotional issues of her own following her failure to save teachers from a fire, and her anti-depressants are causing neurological issues.

Tommy makes a promise to Janet to help her with the baby, but fails to share with her his and Sheila's plans for the future. Elsewhere, Franco checks in on his daughter, Keela, and several crew members plan their exits from the house.

John Dorian, or J.D. as his friends call him, has arrived at Sacred Heart Hospital for his first day as a new intern. He finds that being a doctor is much different and more difficult than he was expecting. He quickly learns that the people he meets are not always what they seem after the first impression.

When the smitten Jonathan suggests an exchange of phone numbers, Sara balks and proposes an idea that will allow fate to take control of their future. Sara asks Jonathan to write his name and phone number on a dollar bill, while she writes her name and number on the inside cover of a used copy of Love in the Time of Cholera. If they are meant to be together, she tells him, they will find their way back into one another's life.
