Joan fires Scarlett, Harry Crane's assistant, when Scarlett has Dawn punch her time card in Scarlett's absence. Furious, Crane bursts into a partner's meeting and questions why Joan has a partnership stake at SCDP, when he has brought in clients and revenue many times over, earning his accomplishments "by the light of day". Head writer of Megan's soap opera, To Have And To Hold, Mel and his wife Arlene, have dinner with Don and Megan and (unsuccessfully) encourage the couple to join them in group sex.

This episode is narrated from Dr. Cox's, rather than J.D.'s, viewpoint. Dr. Cox discusses the hospital and Jordan in his sessions with his psychiatrist, Dr. Gross. Elliot meets Paul Flowers, who she thinks is an attractive and charming doctor, but later learns he's actually a nurse. Carla finally says yes to Turk's proposal.

J.D. and Elliot don't want to repeat the mistakes from their last romantic relationship, so they decide to remain "sex buddies". However, J.D. doesn't feel satisfied with this arrangement. Turk attempts to help Elliot lighten her heavy workload, but his efforts backfire on him. Carla learns the reason why hospitals aren't able to give definite answers.

Turk convinces J.D.'s patient to decide upon surgery instead of medicine, which angers J.D.. Elliot falls for one of her patients, Sean. Dr. Kelso fires two nurses and after Dr. Cox confronts him, he fires 'Coffee Nurse.'
