The film centers around Catholic Irish American fraternal twins, Connor and Murphy MacManus, who become vigilantes after killing two members of the Russian Mafia in self-defense. After a message from God, the brothers, together with their friend David Della Rocco, set out to rid their home city of Boston, Massachusetts of crime and evil; all the while being pursued by FBI agent Paul Smecker.

Rand is revealed to be at a psychiatric hospital, assigned to Dr. Paul Edmonds. Rand continues insisting on his true identity. Harold is revealed to be surveilling the hospital. He has Ward talk to Wing. Ward offers her money in exchange for her confirmation of Rand's alleged condition. Harold secretly visits Rand, who reveals that he is the current Iron Fist, the sworn enemy of the Hand. Wing visits Rand, who reveals his true identity and why Ward is afraid of him. She refuses Ward's offer. Joy deduces that Rand is right and tells Ward, who refuses to free him. Edmonds also learns the truth and asks Rand about after the crash. The latter states that he was transferred by two monks to K'un-Lun, another dimension that becomes connected to the Earth every fifteen years. Edmonds does not believe it and diagnoses him with an anxiety disorder caused by trauma. Harold decides to move Rand to a safe place since he can be useful. Instead, Ward orders his men posed as patients to kill Rand, who overpowers them by summoning the Iron Fist, which he uses to break out of the hospital.

Otto and some of Rye's belongings, including his belt buckle, are buried at the Gerhardt farm. Lou and Ben arrive after the funeral and take Floyd to the station for questioning. She eventually accepts a deal that will absolve her family members for previous crimes they committed in exchange for information about how the North Dakota syndicate runs their drug operation. Meanwhile, Bear intercepts his niece, Simone as she is leaving Mike Milligan's hotel, and drives her to a secluded wooded area where he executes her for betraying the family. Lou asks Karl to stay with Betsy and keep an eye on her and Molly. Betsy tells Karl that she is sure she received the placebo cancer pill instead of the real drug, and wants Karl to look after her family after she is gone. Betsy goes to her father's house to feed the cat and discovers his office filled with strange drawings and symbols. Meanwhile, Headquarters calls Mike saying he has run out of time to take care of things. An "Undertaker" is being sent to take over. When the Undertaker arrives, Mike instantly kills him and his two accomplices, intending to blame the murders on the Gerhardts. Suddenly, Ed Blumquist makes a call from a telephone booth to Mike Milligan, stating that he has Dodd Gerhardt in his car trunk.

Phil and Claire had a huge fight and Phil hasn't a clue as to what it was over, so Gloria steps in to counsel Phil and Jay takes Claire. Meanwhile, Jay may have some regret in buying Gloria a karaoke machine, and Cameron is in full prep mode for a huge fundraising event and enlists help from Mitchell and Luke -- which might not be the best choices.

Hank meanwhile is still reluctant to leave the hospital and go home, even though his doctors have told him he's free to leave at any time. Marie finds an interesting way to get him to agree.

Homer's obsession with Lenny's new plasma TV prompts Marge to enter the family in a sweepstakes, and they win a trip to the Fox Studio Lot. Against Marge's wishes, Homer signs them up for a "Trading Spouses"/"Wife Swap"-style reality show and is forced to take in a controlling wife, while Marge moves in with a beaten-down husband, Charlie, and his perfect son.
