Hazel schemes to appear on TGS; Jack disapproves of Tracy's latest movie.

Doug has invited his friend Ray Barone, and his wife, Debra, over for dinner. Carrie has never met the Barones, and before they arrive at the house, Doug asks her to tone it down a little in order to make a better impression on his new friends. While the guys are left in the living room, Carrie and Debra talk in the kitchen about Doug's comment - and the two find a bond over both having insensitive husbands. Meanwhile, Doug and Ray get themselves into trouble while trying to watch a ball game on TV. The cable isn't working properly, so Doug and Ray try to fix it, resulting in Doug getting stuck halfway between the attic and the bedroom.

J.D.'s brother Dan comes to visit with news their father has died. Dr. Cox has difficulty helping J.D. get out of his depression. Meanwhile, Turk has symptoms of diabetes but in his stubbornness refuses to admit it or visit a doctor as Carla wishes. Molly must employ some advanced psychology to force him to face the issue. Guest-stars Tom Cavanagh as Dan.

After five years working at Sacred Heart, J.D. gets the impression that the same events keep unfolding at Sacred Heart. Dr Cox, returning to work, seems to have lost his self-confidence and Elliot takes it upon herself to help him regain it. Turk annoys Carla by doing the things she cannot while pregnant.

J.D. and Turk save local TV cameraman and become local heroes. J.D. embraces it, but Turk finds all the attention uncomfortable.

When Dr. Cox fumes at Dr. Kelso for firing another staff member due to budget cuts, Dr. Kelso challenges him to balance the budget without firing anyone. Meanwhile, a sitcom writer is admitted, and J.D. starts to fantasize about what it would be like if the hospital were a sitcom, complete with a live audience. Also, Turk and Carla have difficulty trying to keep the romance in their relationship going.

The Griffins join a country club, but end up getting kicked out.

When Homer is unable to take all of his trash to the curb, barely missing the garbage truck, Homer insults the garbage men, leading the family to have their garbage collection service cut off. After the house collects a huge amount of trash, Marge writes an apology letter to the sanitary commission, signing with Homer's name. However, Homer finds out and goes to city hall to rescind it. Following an angry meeting with the commissioner, Homer decides to run for his position. Homer's campaigning fails to win much support, until he promises the town that they will no longer need to do any garbage-related work. This causes Homer to win in a landslide. Initially, Homer is able is keep his promise, but he uses an entire year's budget in a month, causing the workers to go on strike, fearing they will not be paid. Homer makes a deal with neighboring towns to dump their trash in Springfield's abandoned mines to make money, but when the mines are filled, trash starts to burst out of the ground. The town is then forced to completely move five miles away.
