In flashbacks, Rebecca leaves for her tour. Kate encourages Jack to attend Rebecca's first show; after rejecting a colleague's advances at a happy hour, he sets out driving to the show despite having been drinking. In the present, per William's wishes, Tess and Annie plan a celebration of his life, while Beth feels left out; Randall asks her to toast William and she receives a loving postcard William sent from Memphis. Rebecca admits that she kept William a secret because she feared losing Randall; Randall acknowledges that he and William had the time they needed, and he and Rebecca reaffirm their love. Kevin's play premieres and the family loves it, though an important critic fails to attend. Randall quits his job over his firm's apparent disrespect and lack of appreciation, admitting he doesn't have a plan. Kevin and Sophie consummate their relationship and Ron Howard offers Kevin a film role after having seen the play. Kate struggles to tell Toby about Jack's death, saying she is responsible for it.

When a young Amish woman and her son get caught up in the murder of an undercover narcotics agent, their savior turns out to be hardened Philadelphia Detective John Book.

Lainie McCallum (Diora Baird) is raped and cut in her apartment building's stairwell and claims a black lady saved her by punching the perpetrator in the eye. The lady is identified as Nardalee Ula (Saidah Arrika Ekulona), an illegal immigrant from Democratic Republic of the Congo, who came to America after escaping a life of sexual abuse against women used by the country as a "weapon of war". After Lainie dies from MRSA, Nardalee is the only one who can testify against the rapist, but risks being deported. After her successful testimony she is awarded an U-Visa, but declines it, saying she will be going back to the Congo to help save other women from sexual abuse. Cabot announces she will be taking a job with the international criminal court prosecuting crimes of sexual violence in areas of conflict.

Reese must protect Charlie Burton, a school teacher (Enrico Colantoni) who witnessed a Russian mobster shooting an Italian-American mob lieutenant in a Brighton Beach bodega. As Reese and Burton flee the Russians and the Five Families, they stumble into territory controlled by Bulgarian mobsters. Meanwhile, Carter searches for Burton to place him in protective police custody, and in doing so, draws closer to finding "Elias M." - believed to be the illegitimate son of an ageing mobster with a plan to unite the Five Families and reclaim the New York underworld from the Russians. Cut off from Finch as he tries to evade the Russians and the Bulgarians, Reese comes to realise that Charlie is Elias. Elias warns Reese to stay away or face unpleaseant consequences, leaving Reese feeling guilty that other people will suffer because he saved Elias' life.
