Summertime in Chicago, and the Gallaghers are back and up to the same tricks. Frank - after losing a bet with a fellow bar patron - is now in over his head $10,000, and Karen refuses to let him steal the money from Sheila again; Frank uses Liam to win sympathy panhandling, until the bar patron takes Liam as collateral for the money. Lip wants to continue seeing Karen, who is now attending Sex Addicts Anonymous meetings and in a sex-free relationship with a fellow group member; however, he is also preoccupied with summer courses, running a fighting ring, and selling pot and alcohol out of an ice cream truck with Kevin. Fiona and Veronica are waitressing at a local club; Fiona is seeing someone to get her mind off of Steve, though V says her new boyfriend is "a rebound" and "even looks like Steve." Ian is still working for Kash and Linda, whose marriage is deteriorating despite her pregnancy; Ian also expresses interest in attending West Point, and Lip agrees to help him with his application and grades; he is also looking forward to Mickey's release from jail. Debbie and Carl run a babysitting service in the house to bring in extra funds. Sheila is simultaneously trying to find Eddie (who committed suicide last season) and working on being outside her home.

Dexter, using what he's learned from Brother Sam, comes a step closer to the Doomsday killers, specifically Travis. Deb and LaGuerta go head-to-head after a call girl ends up dead. Travis is kidnapped by Gellar for further rituals. After Deb interrogates Travis's sister as part of the DDK investigation, the sister is found dead, posed as the "Whore of Babylon".

Tommy Gavin launches a campaign of dirty tricks against his wife's new boyfriend, and a retired fireman announces that he's gay in a newspaper article, sparking strong reactions at the firehouse.

Debra introduces Robert to her friend Amy. Debra then remarks that Robert is handsome, which concerns Ray.

After TGS goes on a forced hiatus, everyone, except Liz, thinks the words "forced hiatus" means that the show will be cancelled. So everyone, except Liz, gets to work on their Plan B for employment.

Locke's group settles into the Barracks. Sayid makes a deal with Frank to leave the island, in exchange for freeing Charlotte. Sayid holds up his end of the deal; however, he obtains Charlotte by trading her for Miles. Frank, Sayid and Desmond leave the island for the freighter by helicopter. In flashforwards, Oceanic Six member Sayid works as an assassin for Ben.

After seeing Doug eat a hearty breakfast of pancakes and sausage, Carrie advises him to eat a light lunch since they’ll be going to a play with some work friends of hers. Not only does Doug ignore her advice, he participates in an egg roll eating competition during lunchtime. By the time the play comes around, Doug has such an upset stomach that he can’t stop belching. Carrie is infuriated and sends her husband home early only to find him stuffing his face with Yodel chocolate cakes when she arrives. Carrie convinces Doug to go see a therapist who will help him overcome his poor eating habits and lose weight. Doug’s therapist, Dr. Wagner, gives him some great tips on watching his food intake, and Carrie is happily surprised. Soon she starts giving her own suggestions on some of the “problems the doctor can help Doug with . . . like watching too much television, being more spontaneous, tips on being a better lover, etc. It’s not until Deacon gets in on the game and adds some issues of his own to Doug's “therapy list that the truth gets out.

When Lisa encourages Mike to meet with a young female colleague seeking professional advice, Mike hesitates but soon discovers that she is attractive, sports-savvy and able to hang with the guys. After meetings between Mike and his new friend become more frequent, Lisa is prompted to join them, and she and Mike are surprised by her unique proposition. Meanwhile, morning commutes become less solitary for Adam when Callie starts calling him on the way to work, and Ethan attempts to eliminate indications that his relationship with a new girl is becoming serious.

Robert gives Ray a great idea of what to get Debra for Christmas, but is angry when Ray takes all the credit.

Nathan struggles with money problems and reluctantly asks Dan for help. Brooke learns the new man in her life is also her new English teacher. Peyton gets help from an unexpected source. Lucas returns to the Ravens just in time to help the team advance in the playoffs for the state championship. This episode is named after an American folk song covered by Nirvana.

Having just returned from her honeymoon with Meredith, Cristina searches for Burke, but he is nowhere to be found. Meredith, Cristina, Izzie and Alex spend their first day as residents with their own groups of interns. Among the interns are George, repeating his intern year after failing his exams, as is Lexie Grey, Meredith's half-sister whom she has never met. Now that Derek's relationship with Meredith has reached an emotional impasse, he looks to his fellow doctors for friendship; Bailey wrestles with her place within the hospital now that her former interns report to new Chief Resident Callie; and Richard resumes his position as Chief of Surgery.

A health inspector's visit coincides with Bob's efforts to bring in customers.

Cleveland takes the lead in cleaning up the troubled local high school. To earn money, he organizes kids to sell cookies, but the kids start selling drugs instead. When a rival gang steals the kids supplies and takes over their territory, Cleveland takes matters into his own hands to protect his cookie business and end up stealing a duffel bag full of drugs from the gang members.

Ted and Barney talk about a potential life-changing decision together. Robin tries to discourage Marshall and Lily from moving to Long Island.

In the episode, Bart floods the school gymnasium, which prompts the school's principal Seymour Skinner to diagnose Bart with ADD. Bart is prescribed a psychostimulant drug called Focusyn (a parody of Ritalin), and initially starts paying more attention to his studies. After a while however, Bart starts turning psychotic and is convinced that Major League Baseball is watching over the people of Springfield.

When the detectives are called onto a homicide of Jane Whitmore, a journalist who was raped and murdered in her bed with an "X" carved into her cheek, Ned Bogden (William Atherton) is soon the suspect. Cragen discovers that he may be the "Bedtime Butcher" who murdered five women thirty years previously, but the case was closed. ME Warner confirms that he did kill Whitmore and all but one of the women from the '70s, Emily Cutler, as Warner found one blonde hair which was from another woman. As the detectives investigate Mrs. Cutler's murder, they find that her husband, who'd died in a drunk driving accident, had many disputes with three of his female, employees -- all of whom were blonde -- and question Susan Delzio (Jaclyn Smith), the patrol cop who handled the case in 1976. The detectives are able to link the women, Jenny (Susan Anton), Claire (Morgan Fairchild) and Rita (Ann-Margret) to Mrs. Cutler's death. During a search of Rita's apartment, they find a number of old diaries, and mistakenly interpret various diary entries as confessions to the murders of both Mr. and Mrs. Cutler. However, Rita reveals the statement "I killed for you" refers to her having aborted Cutler's baby (which she did at his insistence). Warner does another autopsy on Mr. Cutler's body and found exactly what was in the initial report, that he was burned alive in the car, though it wasn't his body, it was the body of James Rogers, a transient, released from prison the day before Cutler's supposed death. Delzio was the officer that released him, and it is discovered that she helped fake his death and also killed Mrs. Cutler while making it look like the "Bedtime Butcher", so she could finally be with him. Mr. Cutler's former agent was played by (Renee Taylor).

As the students of Springfield Elementary are being driven to a model UN conference, Otto drives the bus off a bridge after his brake pedal is blocked by a grapefruit. To obtain help, Otto swims off, while the kids are still in the bus. Soon, when the bus is nearly full of water, the kids manage to escape and swim to an island. Once there, the kids find no food, causing Bart to swim back to the sunken bus to get the cooler full of food. Lisa advises everyone to not eat much, as it may be the only food they could have for some time. However, the cooler is found empty the next morning, with the food wrappings discarded around Milhouse, who claims a monster ate it. Lisa champions the charter of the Model UN and insists on giving Milhouse a trial, wherein he is found innocent. The verdict angers the kids, however, inciting violence against Lisa, Milhouse, and Bart, who are forced to flee. The three hide in a cave, but are chased out by a boar, which is soon found out to be the "monster". The kids kill the boar and eat it, and a narration assures that Moe saves the kids. Meanwhile, Homer starts his own internet company, but it is eventually destroyed by Bill Gates.
