After seeing Doug eat a hearty breakfast of pancakes and sausage, Carrie advises him to eat a light lunch since they’ll be going to a play with some work friends of hers. Not only does Doug ignore her advice, he participates in an egg roll eating competition during lunchtime. By the time the play comes around, Doug has such an upset stomach that he can’t stop belching. Carrie is infuriated and sends her husband home early only to find him stuffing his face with Yodel chocolate cakes when she arrives. Carrie convinces Doug to go see a therapist who will help him overcome his poor eating habits and lose weight. Doug’s therapist, Dr. Wagner, gives him some great tips on watching his food intake, and Carrie is happily surprised. Soon she starts giving her own suggestions on some of the “problems the doctor can help Doug with . . . like watching too much television, being more spontaneous, tips on being a better lover, etc. It’s not until Deacon gets in on the game and adds some issues of his own to Doug's “therapy list that the truth gets out.

Carrie and Doug remember when they bought their house, and also recall the other house that Doug wanted, the one with the dumbwaiter, right next to the elementary school.

Joe the bartender finds himself in the need of a very expensive standstill surgery that there is no way he'll be able to afford. Much to Meredith's discomfort, Addison requests her as her intern for the day. Still recovering from surgery, Dr. Webber asks George to be his "eyes and ears", but George finds it difficult to pass on any information. Cristina tells Meredith that she is pregnant without revealing the father. When Preston breaks off their relationship, she decides not to tell him either. Izzie and Alex's personal bond grows closer as he shows a more emotional side of himself.

Clare Metcalf, recently admitted to a psychiatric hospital, escapes from the facility's supposedly safe 'time-out room'. There is speculation that the missing girl may have been the victim of an abusive staff member, but when evidence comes in from Clare's boyfriend and then her father, it seems that the truth may lie with a secret Clare has tried desperately to keep quiet since the death of her mother years earlier. When a witness calls in a definite sighting of the missing teenager, the team race against time to find her and stop her hurting herself and others.

The detectives search for a young boy who has been kidnapped by his mother (Isabelle Huppert) in response to the boy's father, Paul (D. W. Moffett), having full custody. The mother is an anthropologist named Sophie Gerard who wants her son to travel the world with her through her anthropological exploits. This orchestrated kidnapping goes wrong, however, and the boy dies in an automobile accident when the kidnapper tries to evade police. This makes Sophie hysterical, and she winds up taking Detective Benson, Paul and ADA Marlowe hostage and shooting ME Warner. It is revealed that Paul was the one behind the kidnapping and the ultimate death of his son.

Ryan is uncertain about what to do, when Wilfred attempts to convince him to stop being selfish and give up a new joy in his life in order for the rest of the people around him to continue on with their happiness at the cost of his own. When Wanda says that her parents are having money problems, Bernie decides to buy her father a car - only to find that no good deed goes unpunished.

Greg is furious that Marcia is dating his rival, Warren Mulaney, who has beaten Greg out for class president and a spot on the first-string basketball team. In retribution, Greg dates Marcia's rival, Kathy Lawrence (who coincidentally beat Marcia out for a spot on the cheerleading squad), even though he'd rather date other girls. Mike and Carol counsel their oldest children about using their dating partners to annoy each other. The family meanwhile builds a "dunk tank" for an upcoming carnival event.

When Bart sees a commercial for the ultimate cruise, he begs his parents for a family vacation. Low on cash, the only way they can go is if each family member sells one valuable item. Once they're away, Bart is determined to make the vacation last forever. So he comes up with a plan to make sure they never have to return home. Guest star: Treat Williams and Steve Coogan.

D.N.A. tests clear Gregory Hensal of abusing his son, but implicate Jake's older brother, whom detectives soon learn has been running his own personal Internet pornography site. T.A.R.U. tech Ruben Morales joins the detectives in their search for Teddy after he goes missing, but his own guilt about his nephew's molestation after being raped by an online predator he met using the computer Morales had given him colors his judgment when dealing with one of the suspects.

After seven young women in a six-month time span are raped by a flash and dash assailant on the city subways, the brass calls in a psychiatrist to help the team get into the head of the perp. When he is caught, all seven victims identify him in a police lineup, but the IDs are suppressed. However, the detectives find that he possesses the driver's license of a new victim who could put him away.

The Chief of Staff, now in a coma, begins wandering the hospital as a spirit.

Robert and Debra become infatuated with swing dancing.

The episode centers on Sandra Clark who has moved to New York City to work in the film industry, but leaves when she discovers that the film only specializes in porn. She gets a job working at a spa the next day, where she meets Stephanie (Debra Stricklin), whose boyfriend is dating someone else behind her back. Sandra knows and worries that if she tells, Stephanie will not speak to her anymore. Stephanie does not believe or speak to her. But with the help of her old friend, Pearl Shay from 227, Sandra exposes him as a liar and a cheat. Stephanie cuts him loose and she and Sandra reconcile.

When Roger is rejected by Steve, he heads out in search of a new human companion. But when his new friend starts to abuse Roger, Steve and Hayley plot to get Roger out of the bad relationship. Meanwhile, Stan and Francine team up to save the Mr. Pibb factory after news hits that Mr. Pibb is going to be discontinued.

All six children have the measles. Carol calls the girls' usual doctor, a woman, while Mike has called the boys' doctor: a man. The girls prefer their usual doctor, and specifically a female doctor, the boys prefer their doctor, and specifically a male doctor. Mike and Carol compromise to retain the services of both doctors, who have meanwhile decided to combine their practices.

The Professor develops a phosphorus dye which Gilligan drinks and becomes a human lighthouse bulb. Gilligan helps the Skipper lose enough weight to get back into the navy once they are rescued. Also, they help Gilligan gain weight so he too can return to navy life.
