In Springdale, Washington, a teenage girl taking pictures of herself is killed in her own bedroom by what appears to be Thinman (a pastiche of Slender Man), some kind of monster that appears in various videos and pictures of unnatural deaths. Sam and Dean find their old ghost hunting rivals Ed and Harry of the Ghostfacers are also there investigating, collecting more stories on Thinman. Sam and Dean are confused because the deaths don't seem related to the supernatural and the pictures seem faked, but then Thinman is caught on-camera murdering a diner manager. After Harry himself narrowly escapes Thinman, Ed confesses that he had invented Thinman to keep Harry from leaving to lead a normal life as their old teammates had, a revelation that infuriates the other man. Sam and Dean are taken captive by the local deputy, who reveals that he and his psychopathic partner-in-crime, a busboy who had killed his boss the diner manager and the girl for petty reasons, have been using Thinman as a cover to go on a killing spree, disguising themselves as Thinman and staging the murders as supernatural occurrences to make it seem that Thinman is real in order to feed the legend. They plan to have "Thinman" kill Sam and Dean next, but when Ed and Harry arrive, they get distracted, allowing Sam and Dean to break free and fight back; in the ensuing struggle, Dean kills the busboy and Harry is forced to kill the deputy to save Ed. Dean then covers up their involvement by staging the scene so it looks like the killers killed each other. At the end of the episode, Harry refuses to forgive Ed for his deception and leaves the Ghostfacers and Ed behind for good.

With her intuition and the more subtle signs of a heart attack, Bailey winds up in the ER at Seattle Presbyterian; however, after her doctors think that she's making it up and really just suffering from her OCD, she calls on Maggie to come save her. When Maggie arrives, she runs into Richard who found his way there based on Bailey's record of attendance. With her life flashing before her eyes, Bailey reflects on her upbringing and her past as a surgeon with all the stressful fights and struggles she's gone through to get to this point in her life. Right before she goes under for surgery, Bailey asks Maggie to call Ben who is able to get there before she wakes up. Ben announces that he's quit being a firefighter, but Bailey demands that he go back and never do anything other than what he truly loves.

A new job puts Taystee close to Caputo. Lorna has to get creative in her marriage. Soso and Poussey deal with some awkward truths.

agrees to a weekend camping trip but struggles to fit in with Nathan and the guys. Back in Tree Hill, Brooke and Haley spend Girl's Night doing a little bonding of their own, and Quinn helps Clay come to terms with a tragic loss. Meanwhile, Dan and Rachel's past catches up to them.

Aidan approaches the old woman he has been paying for her blood, but she says she can no longer give any blood but offers her daughter instead for him later. At the hospital, Josh is approached by Brynn, another werewolf, who needs him to break her twin brother Connor out before the next full moon. Sally tries to help Zoe flirt with the new surgeon, Dr. Tim Forrest, to no avail, and Zoe invites her to come to the support group she holds for ghosts. Aidan goes back to drink from the old woman's daughter, but when he discovers that she is a young child, he is revolted and runs off. He is called in by Suren to consult with a proposed addition to their group: a local mobster. Aidan believes he will not be an asset, despite Suren's insistence after the mobster brings them one of Bishop's orphans. Josh is approached by Brynn and Connor who take him out to a club, revealing that they are purebred werewolves, rather than having been turned. When Josh mentions he is seeking out a cure, it piques Connor's interest, and later when he mentions he is trying to find the biological switch that initiates the transformation to turn it off, Connor and Brynn reveal that they want to become the wolf permanently. Josh is appalled at their wishes, but with their funding he may be able to solve the problem, and asks Nora to come meet them. At Zoe's support group, Sally meets up again with Nick, at which Zoe is surprised that they know each other. After accepting what seems like a date, she talks with Zoe the following day who reveals that she is seeing Nick intimately. Nick later confirms this, and his newfound ability to avoid reliving his death day after day after meeting Zoe, which leaves Sally heartbroken. Later, while idly watching Dr. Forrest and his girlfriend Janet being intimate, Sally takes the opportunity to possess Janet so she can feel a loving embrace. While mulling over the mobster's inclusion as a vampire, Aidan has flashbacks to 1930 when his protege Henry started to want more out of his immortal life, specifically what Suren would give them. When Aidan turned down Suren's advances, Henry took up the offer and became Suren's lover. In the present, he tells Suren that he has made up his mind and will turn the mobster. As he begins drinking the mobster's blood, he has another flashback before he starts draining the mobster dry. He proclaims that this time he will have Suren, who begins feeding on the mobster before the two vampires make love to each other.
