Xiao Ping Li, a scientist involved in a chemical weapons project known as White Fog, is drugged at an immunization center in China and whisked to Washington, D.C. by CIA agents for interrogation. The Pavlovich brothers specialize in abductions of high value targets, including General Ryker's daughter. According to Red, they are in town planning their next hit, Li. Meanwhile, Tom discovers that Elizabeth knows his secret, informs his people and flees. While the team tries to locate the Pavlovich brothers' target, Red makes a deal with the Brothers to abduct and deliver Tom to Elizabeth. Elizabeth tries to torture Tom into revealing his bosses, but he quickly turns the tables and corners her. After explaining his job never included hurting her, he claims to be "one of the good guys" and tells Elizabeth that Red isn't at all who he seems to be. Before fleeing, Tom tells Elizabeth about a safe deposit box, the key to which she already found in the lamp. With Tom in their view, Red instructs Dembe not to capture him again, but rather follow his tail. The next day, Elizabeth opens the deposit box and is shocked when she looks at the photo inside.

Mark invites Sarah to a friend's engagement party which stirs up a serious conversation about a baby in their future. Meanwhile, Amber receives a job promotion but questions her own qualifications, Adam and Crosby clash over business strategy for The Luncheonette and Zoe's behavior causes Julia and Joel to question their adoption agreement.

Josh looks at Heggeman who has him in his sights when Nora, fully transformed, attacks Heggeman and kills him. Aidan digs up Suren, Mother's daughter, and brings her to the Halloway Hotel so she can feed on willing people so she may regain her strength. Sally, horrified at the actual sensation she had in her nightmare, seeks solace with Stevie and his friends Dylan and "Boner", who Josh and Nora meet when they get home. Josh reveals to Aidan that Nora has killed Heggeman. Nora tries to clear her head as Sally reveals to Stevie she physically felt something when she opened the Door in her dream. Aidan attempts to help Suren take her place as Boston's head vampire by suggesting that they install a new vampire police officer as their assistant, but she has other plans, having chosen the still human police officer Cecilia to pick a new subject rather than one of Bishop's sires. Suren tries to get Aidan to turn her, but he refuses. Josh tries to console Nora at work when he is reminded that he has to attend the first-year med student mixer. Josh later manages to convince her to go, where she gets drunk to take solace in the fact that she is now a werewolf and that she has miscarried her and Josh's child. Dylan convinces Sally to go to a party to "Rage", where Sally discovers that ghosts can take over living people's bodies if they are willing or in a weaker state of mind. Sally relishes in the fact that she can eat, drink, and feel things, again, until Dylan tries to make a move on her in their stolen bodies. Sally refuses and Stevie pulls them out of their bodies to help her, before causing Dylan to disintegrate entirely. Josh decides to drop out of medical school, and try to find a cure for his and Nora's condition instead. Aidan discovers Suren has turned Cecilia, and she tells him he will do everything to make her the best so he can be free of Mother. Sally fears that she now knows that ghosts can permanently destroy each other, but Nora convinces her that it is better to know what one is capable of, with her remembering that she did kill Heggeman as the wolf.

Ben returns and Bailey is skeptical of his arrival, until Ben realizes that she has finally moved on. A stampede at a comic convention prompts for the ER to be inundated with colorful costume-bearers.

Annie and Reva are captured near the Polish-Belarusian border by a man (Peter Stormare) working for the secret Soviet police when they try to deploy surveillance cameras in the Bialowieza Forest. Meanwhile, Jai assembles an extraction team to rescue them in Belarus.

Against orders, Beckett travels to Los Angeles while investigating the murder of her training officer Michael Royce, last seen being arrested in the Under the Gun episode; Castle questions Beckett's motives.

Crosby keeps Jabbar overnight while Jasmine has a dance audition; he has to take Jabbar to the emergency room when Jabbar he gets his thumb stuck in a soda can. Adam and Kristina notice that Haddie's cell phone bill is ridiculously large and seems to be concentrated to one particular number; upon further investigation (including sneaking into her room and enlisting Crosby to hack into her computer), they find that she has a boyfriend. Sarah has to have the "talk" with Drew, and rejects her father's help. Amber babysits Sydney while Julia and Joel have a date night while out they run into Racquel and her husband who are also on a date night with their daughter. Sarah continues to bond with Amber, and Adam and Kristina learn to accept their daughter's newfound independence.

House and the team take on the case of a young woman who, along with her husband, is rescued at sea en route from Cuba in a desperate attempt to personally see House and get a diagnosis for her illness. During her stay in the hospital, she develops a new symptom: her heart stops - but she miraculously keeps talking. Foreman prepares for his last day at Princeton-Plainsboro Teaching Hospital. In the end of the episode Chase is fired by House when he tries to confront him, but surprisingly Cameron also resigns, leaving House without a team.

Nate and Lisa take Maya camping; Ruth enjoys spending time with new friend, Arthur; Claire and David talk about sexuality.
